Post-Rehabilitation programs support individuals to maintain their journey to living a  physically, mentally, and emotionally healthy life. 

These classes are developed and delivered with our community partners in the health sector and facilitiated by trained and certified intructors. 

Respiratory & Cardiac Maintenance 

Respiratory & Cardiac Maintenance is a community-based exercise program delivered in partnership with Lung Health Foundation for people with cardiac or pulmonary conditions (ie. Heart attack, Angina, COPD, Pulmonary Fibrosis, etc) to continue engaging in an exercise routine following a rehabilitation program. This program aims to help participants maintain muscular strength, flexibility, and activities of daily living.

Virtual Fitness For Breath 

Virtual community-based exercise program delivered in partnership with Lung Health Foundation for people with pulmonary conditions (ie. COPD, Asthma, Pulmonary Fibrosis, etc) to aid in maintaining fundamental movement skills during activities of daily living.  

Durham Dance for Parkinson’s® 

Community-based program created by Dance for Parkinson’s, to aid in the management of symptoms and maintain or improve gait and balance through dance movements. This program offers an inclusive community to target your mind body connection and lift your spirits through the joy of movement and music. Care partners, friends, family, etc are encouraged to participate.

Total Knee Rehab 

Community-based exercise program created for those who have had a knee replacement or knee injury, to continue an exercise routine following a knee rehabilitation program. This program aids in maintaining or improving strength of muscles supporting the knee, balance, and range of motion.

Minds in Motion®  

Community-based social program created by Alzheimer’s Society of Durham Region. Minds in Motion® incorporates 45-60 minutes of physical exercise and 45-60 minutes of mentally stimulating activities for individuals living with dementia and their care partner. for individuals living with early to mid-stage Dementia and their care partner.

Living with Stroke

In partnership with Heart & Stroke Foundation, created to help those who have had a stroke gain knowledge and confidence while completing an 8-week facilitator led program. Participants and their caregivers will learn about goal-setting and have the opportunity to share experiences while supporting one another.

The Healthcare Professional Referral Form must be completed by your healthcare professional or medical doctor. Once the form is completed and signed, please return to Abilities Centre to finalize your registration.

Are Post-rehab programs for you?

  • Respiratory condition

    Anyone living with a chronic respiratory condition is encouraged to participate (COPD, pulmonary fibrosis)
  • Heart condition

    Anyone who has experienced a cardiac event/is living with a cardiac condition, is encouraged to participate (history of heart attack,  arrhythmia)
  • Physician referral

    Must have a physician referral completed prior to starting in the program  
  • Age 14+

    Due to the equipment used in the class, we ask participants to be at least fourteen years of age 

  • Improve or maintain your physical workout

    A safe and supported program that empowers respiratory and cardiac clients to continue improving through exercise
